Vietnam reports first tin poisoning death

A 35-year-old man working for a curtain manufacturing company has become Vietnam’s first recorded death from tin poisoning.

Dr.Nguyen Trung Nguyen, director of the Poison Control Center of Bach Mai Hospital in Hanoi, said on Thursday the hospital had received six patients severely affected by tin poisoning, including one who died.In all cases, blood tin concentrations were 10 times higher than the permissible limit, marking the first case of acute tin poisoning reported in Vietnam.

The deceased and five other colleagues were working at the recycled plastic fertilizer division of a curtain manufacturing company in Thanh Mien district, Hai Duong province, northern Vietnam.

Before starting work for the company, all six were in good health.But very soon, from 4 days to 1 month, the workers all had common manifestations: psychosis, especially memory loss, confusion, agitation and abnormal behavior.

The deceased man, whose name has not been released, was hospitalized on July 9 with all of the above-mentioned symptoms.He went into a coma soon after and died.Specific details of the death, including when it occurred, were not released.

Magnetic resonance imaging (MRI) showed that the deceased's brain had diffuse white matter damage, metabolic acidosis, and severe hypokalemia (abnormally low blood potassium).At first, his family suspected he suffered from mental problems because his father had just passed away and he had some problems in his personal relationship.

His relatives told the doctors that some of his colleagues were also experiencing similar symptoms, so the doctors advised the workers to take the workers to the hospital for an urgent check-up.

A 42-year-old worker was later hospitalized in an unusual mental state, with symptoms of dementia, driving and wandering aimlessly around the village.Test results revealed serious blood disorders.

Nguyen said doctors at the hospital researched medical information, consulting international experts and colleagues as they tried to diagnose the problem.When advised to check the blood tin concentration, the Institute of Chemistry under the Vietnam Academy of Science and Technology conducted the test.

The results showed that his blood tin concentration was more than 200 micrograms / liter, more than 40 times the allowable threshold.

The patient was given a regimen of dialysis and detoxification with tin.After a week of intensive treatment, his condition improved and his memory gradually returned.

Blood samples from a 35-year-old deceased patient showed tin concentrations 50 times higher than the safe level.

Of the six cases, some had no typical symptoms, but testing revealed severe hypokalemia, metabolic acidosis or brain damage, and significantly high blood tin levels.

Doctor Nguyen said that the patients were poisoned with organotin compounds.Tin exists as a metal atom, including inorganic tin compounds and organic tin compounds.Metallic tin and inorganic tin are essentially non-toxic.

The organic compounds of tin are highly toxic and readily absorbed through the skin, respiratory system and gastrointestinal tract.They are used as a heat stabilizer and resin.

There is currently no specific treatment regimen for tin poisoning, a new occupational health problem in Vietnam.In the world, only a few cases have been recorded and the risk of tin poisoning comes from plastic recycling and exploitation.

Bach Mai Hospital reported the incident to the Ministry of Health and related agencies to track down the source of poisoning and take measures to prevent recurrence.

The Centers for Poison Control has urged people who work in high-risk conditions for tin poisoning to urgently get tested.

Hai Duong Provincial General Hospital and the Provincial Center for Disease Control have just announced that, out of 15 people who were examined, 8 had symptoms of tinnitus, hearing loss, and 6 had hypokalemia.They have been asked to be hospitalized for further evaluation and treatment, but none have so far, leaving medical professionals worried about the risk of tin poisoning.
